I give permission for my student/myself to be transported to official off-campus Trollwood activities such as inclement weather relocation, field trips, and/or promotional performances. I understand, acknowledge and agree that:

The Fargo Public School District will provide for reasonable supervision of students within its care and control. The supervision will be consistent with the ages of the students. However, the District is not an insurer of the safety of the students nor can it supervise all movements of all students at all times.

There are some instances where students will use their own transportation for Trollwood activities. There are certain risks inherent in travel. The District does not carry insurance coverage for accidents occurring while students are driving in personal vehicles. I understand that if my student, or myself, is injured in a motor vehicle accident while being transported in a vehicle not owned by the District, the District will not be responsible for any injuries or damages that result.

Parent/guardian and student will carefully read all Trollwood policies and procedures on website. No registration will be processed without a signature, accompanying registration fee payment, and payment option selection. By signing below, I acknowledge and agree that: (1) I have read and agree to abide by all Trollwood Policies and Procedures. (2) I agree to pay all registration, program, and other applicable fees in full (unless funded through scholarship and/or financial aid).

Media Release Statement. My signature below provides consent to Trollwood for the use of media images, including but not limited to photographs, film, digital images and video, interviews, and survey responses involving my student/myself for publicity and marketing purposes for the District. These may appear in, but are not limited to, publications, presentations, advertisements, websites, or media showings of Trollwood programs and performances.
